It is stunning in the way it evokes a mood.I think the worms from the chest are ones destiny (he follows it to find the gun that eventually leads to his own demise), and if you are really perceptive you can see them...they lead you through your destiny. I think the movie was pointing out that sometimes individuals who seem to be crazy, are really just ultra perceptive. Like the lady who figured out time travel and the rabbit friend who was shot. They all knew about how to manipulate time. And his trip becomes more intense when he stops taking his meds.
Darco found a wormhole and traveled back in time to erase his own existence and thus save his beloveds life (she never met him). That is why they never found the plane at the beginning of the movie...the engine came from the future. Of course this offers a paradox, but it gives lots of interesting things to ponder...like an intense mind puzzle that can't be solved.
Maybe the message is that one can't avoid destiny.
People compare it to American Beauty, but I find that only the film style resembles it in any way.

I too was moved by this story. I agree that Donnie travelled back through the portal to save his girlfriend. I think he also went back to "save the world", to knit back together the rent in the time-space continuum created by the "object" (the jet engine). I'm not sure I understand why he was laughing just before the end.The movie is beautiful, funny, and disturbing. The special features and the website:
