They're noisy. If your equipment is located in the HT room, you might find this to be problematic. With judicious purchasing (PC Power and Cooling makes lower noise Power Supplies, Cases and Fans) you can cut this down some. Also, some of the HTPC vendors already consider this and build appropriately out of the box. 7200 RPM hard drives which are the norm these days are terribly noisy and you don't want to know about 10K RPM drives :-(

They still aren't appliances and can crash from time to time from hardware and software perspectives.

I'm not meaning to sound like I'm discouraging you from this choice, but you need to consider these items and whether you're willing to accept the additional effort required on your part.
