Is it fan noise or hard drive noise?
If it's a fan, maybe you can fit a higher-quality fan, or a bigger but slower fan. Or maybe slow the existing fan down; stick 3 or 4 1N4005's in series with the power lead and you should see a big reduction in noise. If the fan blows through slots or perforations in the case, cutting them out and installing a simple wire grille will be quieter.
If it's the hard drive... it might be possible to clone the drive over to a different and quieter model. Check some PVR forums for advice on that kind of thing.
