I was watching Napoleon Dynamite with director's commentary the other day, and in one scene where his girlfriend is talking on a payphone, he commented on how you can see another actor in the steel payphone's reflection. He was wearing an outfit that you can see in another scene that they shot on the same day. Interesting. With big budget movies these days, you probably won't see such errors, as a sharp-eyed editor will find reflections and remove them digitally. One of my favorite shots in the original Matrix, is when Morpheus is talking to Neo just before Neo takes the pill, and you can see Neo's face reflected in Morpheus' sunglasses, though he's looking straight at the camera. Cool effect.
