For me it was deciding he was still a functioning member of the human race, or if our protagonist was the narrator of his own delusional fancy.....including the hallucinations.....suggesting that he'd been in his bed at the mental hospital the entire time.
The last scene where he leaps out the window...? Hallucination or real, take your pick. If he really did jump out the window, I'm pretty sure he didn't fly away.
