We have had that for almost 2 decades now: HBO.Sorry about the smart-ass comment - I couldn't resist.
But seriously, I doubt pay would pay what it costs to have all their programming commercial-free. We would also have to get used to new schedules (timetables) for our programming in 20 min blocks instead of 30min.
