How are you managing behind TV/inwall wiring?

TVs, VCRs, DVD players, Home Theater systems and more.

I am preparing to get a 37-42" LCD. How do I prepare the wall behind where the TV is to be hung with regards to a panel for all needed electrical/A/V/cable/etc. connections? What is a reasonable size/depth for such a panel?
