The simplest approach, if you have an AVR that does HDMI switching and can accept audio from its HDMI input, is to route the signal through the AVR. My Denon 2807 handles that approach very well and it does not pass the audio signal to the TV unless you choose to have it do so in the setup menus.If you don't have an AVR that provides that functionality, then you're going to have to run a separate audio feed to your AVR/amplifier or use an external switch box. If you use an external HDMI switch box, a lot depends on what switching capabilities it provides. If it has only 1 HDMI out, that is going to be to the TV and you're going to have to run a separate audio feed from the source to the AVR/amp. If it has a second HDMI out, you could run that to your AVR/amp if it has HDMI inputs.
You may well be able to take the audio from the TV's audio outs and run that to your AVR/amp but the audio outs from the TVs I've seen are 2 channel and that won't work particularly well for you if you're using a surround sound system because you're going to lose the separation between some channels in the process and then use something like Dolby ProLogic in one of it's several varieties to recreate multi-channel surround info. Better to run multi-channel info directly from the source to the AVR/amp if the source has multi-channel analog outs.
