You were expecting Borat? Oh no...This film is comedic in the sense of black comedy, but it it isn't intended "comedy" in the traditional Hollywood mode, more like Catch 22. The humor is ironic and black, not laugh out loud. The intent is serious.
As you may have noticed, the title character is not an especially nice guy...but that's a key part of the message, doncha know, if you can get that far. BTW, the director took many of the incidents in the film directly from actual experiences in the medical quagmire.
An excellent film nonetheless, and one of my top films of 2006.
Start it over maybe and give it another chance. Haven't seen it on DVD but did catch it in theaters last year.
